OVERVIEW OF ASD HEALTH CLINIC
There
are two nursing offices on campus staffed by qualified registered nurses during
school hours.
To
maintain the health and safety of the students the following completed forms
are required upon admission:
- Student Health History Form
- Student Physical Exam Form
The
following minimum immunization standard is required for all students attending
ASD:
- BCG OR mandatory Mantoux testing (results from within previous twelve
months) if no BCG vaccination
- OPV/IPV (Polio)
- DPT (Diptheria, Pertussis and Tetanus)
- HBV (Hepatitis B vaccine)
- MMR (Measles, Mumps, Rubella)
(Copy of
immunization records required upon admission)
